Office Policies
Payment and Insurance Payment or co-pay is expected at the time of service. We accept cash, personal checks, Visa and MasterCard. If your visit is covered by Indemnity Insurance, you will need to pay and then submit your forms for reimbursement from your insurance company.
You will need to bring your Membership ID Cards for all your insurance (primary and secondary) and any Workers’ Compensation or No-fault information necessary for your visit (case #, company, etc.).
If you are referred for a consultation with one of our specialists by your primary physician, you will need to bring a consultation request. Please call to confirm that we have received copies of your medical records, test results, and X-rays from your primary physician and to check if there is anything you will need to bring with you for the visit.
Forms For Your Initial Visit On your first visit to our offices, you will be asked to complete several forms. If you wish, you can download the forms, complete them at home, and bring them with you when you come for your visit. This will save you time at registration. Prescription Refills Refills can be filled by letting us know 3 business days in advance. Please call our automated refill line and let us know what medication to refill and which pharmacy to call or where to mail the prescription form.
After Hours When the offices are closed, we have a physician “on-call” 24 hours a day 7 days a week to help you with emergencies. You should go to your local hospital emergency room for any serious emergency situation.
Visitor courtesy: Perfumes/cologne When visiting our office please avoid using heavy perfumes/cologne that could negatively affect other patients/staff.
